Okay, I sure took some time to write my comment. But as promised here is my theory.
The funny thing that I remembered when thinking of your story is a real crime that happened in japan in the 80s when a high school girl got kidnapped and no one was able to find her for over 40 days.
During that time she had to endure all types of torture (You can imagine all sorts of horrifying things). After that period her corpse was found buried in an abandoned house.
I know you're asking why am I saying this. Here are the reasons:
- The name of the victim was Misaki.
- Some of her classmates were part of the group that kidnapped her.
- strangely, no one asked or wondered where was she during that period. Her family didn't report to the police that she was missing. They thought she just run from home during the first month.
Unlike your story, the criminals were found after a short period and were sentenced to 9 - 10 years of prison. (A very light punishment if you ask me)
After passing half of the period, one of her classmates intentionally destroyed her grave thinking that she was to blame for his wasted life.
If I compare this story to yours I would never think that Kazuya is one of Misaki's classmates who locked her in the school.
Now, we have two other possibilities:
1/ Kazuya and Hotaru are ghosts of students who got trapped in the school years ago.
This possibility is very unlikely to be correct since Hideki and the others recognized them since their first appearance.
2/ They're classmates of Hideki
It's the best answer that I could've found.

Kazuya can be the son of one of the criminals (possible)
Kazuya isn't a ghost since the ghost that was helping them (sorry I forgot her name) said that she was the only ghost that was able to manifest.
Kazuya transferred to this school shortly after Hideki did (not mentioned in the story but it can change a lot)
