Because of Itsuki's ability, he can perceive someone's emotions, but his ability only tells him what emotion they're experiencing, not why they're experiencing it or what that emotion means in context. So when she tells him she "likes" him, he's confused about why she felt the need to tell him. He can already sense her emotions, but he doesn't automatically connect those emotions to the romantic meaning of her confession. To Itsuki, "liking someone" doesn't inherently carry the same romantic meaning that it does to most people.
