You were spot-on that the visual cues were confusing, but I’m glad you picked up on the two distinct voices! This novel uses a ‘dual narration structure’ :
> Default Narrator (3rd Person Limited): This voice recounts the main events, such as when it states, "He lost his life."
> MC-as-Narrator (Haruki/Flux): This is the self-aware voice in the [Bold] text that breaks the 4th wall and acts as an unreliable narrator, speaking directly to the reader. The —italic— remains his inner thoughts.
