It's an interesting way of seeing it, lol. However, that kind of "narcissism" could be seen as his own self-centered world, never gazing at what is most near - which I find quite fitting for his current state.
And yes, you're absolutely right, Athisia's personality is quite something to behold - but unlike most studied PTSD, his is more subtle, less angry, yet all the more devouring, all the more haunting. With it, he is emotionally stunted, his sense of empathy and compassion crumbling away; so, the only thing left is that quirky smile.
Maybe it is a sign of his last vestige of humanity, maybe it is a coping mechanism, maybe he doesn't know anything else other than that smile? But do I, the author, even really know the truth? Sometimes, his personality just really writes itself without my input : D
